A CAPA report in IT operations is a corrective and preventive action record: for each root cause established after an incident, it lists the action that fixes the immediate cause, the action that prevents recurrence, the named owner, the due date and the evidence that the action was completed. The term comes from quality management and regulated manufacturing, and it applies to IT wherever an auditor asks what was done after an outage and whether it was actually finished.
