Blameless postmortem

A postmortem practice from the SRE tradition in which the review focuses on systems and conditions rather than individual fault, so that people report honestly. It is compatible with rigorous RCA: change attribution should state what changed rather than assign fault to a person, and the causal chain should describe conditions that allowed the change to cause harm.
