Blameless postmortem
processA postmortem practice from the SRE tradition in which the review focuses on systems and conditions rather than individual fault, so that people report honestly. It is compatible with rigorous RCA: change attribution should state what changed rather than assign fault to a person, and the causal chain should describe conditions that allowed the change to cause harm.
